This proposal details the difference between two mast cell conditions mastocytosis and mast cell activation disorders: primary MCAS, secondary MCAS, and idiopathic MCAS. The diagnostic criteria for MCA has new robust diagnostic criteria to reduce misdiagnosis: clinical symptoms, tryptase levels, other mast cell mediators (histamine, prostaglandin D2, urinary metabolites), and response to treatment.

Clinical Approach to Mast Cell Activation Syndrome: A Practical Overview. Journal of Investigational Allergy and Clinical Immunology, 31(6), 461–470.

This review speaks about the diagnosis of MCAS with 3 criteria: clinical signs/symptoms, tryptase levels increase of >20% within 1-4 hours after an acute attack and response to anti-mediator therapy. The authors explore the assessment of clonal bone marrow mast cells as a diagnostic tool and focus on the evaluation and treatment for suspected MCAS.

This review reports on mast cells and their role in fibromyalgia, with a theory that mast cells in the hypothalamus contribute to pain and inflammation by releasing neural sensitizing molecules. Presumably, these molecules may directly stimulate nociceptive (pain) neurons or through the microglial cells. Preusmably, inhibiting mast cell stimulation could reduce pain and fibromyalgia symptoms.
